Protecting your health and the health of your community should be all the incentive you need to take the Daily Symptom Survey, but we’re adding one anyway: a weekly sweepstakes for students and employees with ucdavis.edu email addresses.

The survey is required before entering any campus facility; each day you take the survey, you will receive an automatic entry (one per day), during the sweepstakes period, Sept. 28-Dec. 11.

Every Friday for 11 weeks, Oct. 2-Dec.11, four names will be selected at random to win $100 UC Davis Stores gift cards.
